Does the nipple have to be completely removed during breast reduction surgery?

Generally the nipple is not removed during breast reduction surgery. The technique will vary from plastic surgeon to plastic surgeon depending on how comfortable they are with the procedure. Detaching the nipple could potentially lead to unfavorable complications, and should be avoided at all costs.
