Facelift surgery will not stop the aging process. It will slow the process but not stop it completely. Facelift surgery is not meant for the following:

Correction of forehead lines

Correction of under eye fullness/loose skin

Will there be any scarring with a facelift?

As with any surgery, there will be scarring. Placement of the incisions varies as to the type of facelift that is performed. Incisions can be within the hairline or in front of the hairline, in front of the ear and/or behind the ear. These incisions are typically well hidden and fade very well, however scarring is unpredictable. If scarring is a concern of yours, discuss incision placement with your Pittsburgh plastic surgeon. Who is an ideal candidate for a facelift?

What can I expect to look life after a facelift?

You will go home with your head and neck wrapped in dressings, which are usually removed 3-4 days after facelift surgery. You can expect some swelling and bruising that could last 2-4 weeks.

What is the recovery time for facelift surgery?

What can I expect to look like after rhinoplasty surgery?

There will be a splint placed on your nose after rhinoplasty surgery. The splint will be removed approximately 1 week post-operatively. You can expect some bruising and swelling immediately after surgery. What is a septoplasty?

Septoplasty is the procedure which realigns the septum (the room divider between each nostril). If your septum is crooked, it will obstruct one of the airway passages and place constraints on air flow through the nose. A deviated septum can cause medical problems such as difficulty breathing and aggravated sinus issues. What if I had a nose job long ago and never like it?

If you had a previous nose job (rhinoplasty) you have the option of a secondary rhinoplasty. Re-operation seems to occur more commonly in nasal procedures than in other cosmetic surgery operations because we cannot always predict how the nose will heal over time. Scarring is the main tradeoff with breast reduction surgery. The scars encircle the areola, drop vertically from the areola to the crease beneath the breast, and run horizontally in the crease. Any of these scars may thicken and spread or remain red for many months.

Breast reduction surgery is covered by insurance companies as long as medical necessity is established. Each insurance company has its own set of criteria that must be met before they will issue authorization to perform the surgery. Insurance companies will look at symptoms (pain, rash, etc), as well as your height, weight, and bra size. How can capsular contracture be prevented?

It is still not known if capsular contracture can be prevented. We recommend massage to the area of the breast implants at a specified time, well after healing is completed. The thinking behind breast massage is to keep the capsule as soft and loose as possible. Because we do not know why capsular contracture happens, we can’t guarantee that any type of prevention will be successful. Dr. Barber will advise you as to the best options for decreasing the risk of capsular contracture.

What is capsular contracture?

Capsular contracture is a complication that can occur after breast augmentation surgery. During surgery, Dr. Barber creates a pocket for the breast implant that is somewhat larger than the implant itself. During healing, a fibrous membrane called a capsule, forms around the device. Under ideal circumstances, the pocket maintains its original dimensions and the implant “rests” inside, remaining soft and natural. For reasons still largely unknown, the scar capsule shrinks or contracts in some women and squeezes the implant, resulting in various degrees of firmness.

Capsular contracture is not a “health risk”; however it can detract from the overall results of breast augmentation surgery. Rarely, capsular contractions require additional treatment/surgery. Dr. Barber will advise you of the options for decreasing your risk of a capsular contracture.

Should I be concerned that the breast implants will rupture?

Saline breast implants are made of salt water. If for any reason the valve or implant covering fails, the saline will leak and be excreted by your body. This causes no medical harm, but the implant will need to be replaced in a secondary procedure.

Silicone implants are now made with cohesive gel. MemoryGel is Mentor’s proprietary silicone gel that holds together uniformly. When you cut a MemoryGel breast implant in half, the material holds together uniformly (no leaking).

When can I start exercising after breast augmentation surgery?

It is important to follow your Pittsburgh plastic surgeon’s specific advice before resuming your exercise program after any surgery, specifically breast augmentation. An increase in blood pressure after surgery could result in bleeding under the skin which may delay healing and potentially affect the results of your plastic surgery.

The first 2 weeks are critical for breast implant positioning and appearance. Anything that would put stress on the chest muscles (heavy lifting) or bouncing of the chest area (aerobics/jogging) could cause the wound to break open, the internal stitches to break or the implants to shift. Once again, these actions may affect the final outcome of your breast enhancement surgery. Gentle walking is permitted in the first 2 weeks, as long as you are feeling well enough. Strenuous activities may be resumed, with Dr. Barber’s permission, approximately 3-4 weeks after surgery. Intense upper trunk exercises (curls, bench presses, push ups, etc) should be avoided for approximately 4-6 weeks after the procedure. Once you resume your exercise routine, start slow and listen to your body.

Textured breast implants are a coated breast implant with a rough texture as opposed to smooth implants. What is the difference between round and contoured breast implants?

Contoured breast implants have more of a tear-drop shape. Round implants will flatten like a natural breast when lying down and form a tear-drop shape when upright. Contour implants will continue to hold their shape when lying down. Also, contour implants have been known to rotate in the pocket making the heavy end on the superior aspect of the breast giving it an unnatural appearance. What is the “silent scar” technique?

A silent scar technique merely means an incision that is placed in a hidden area during your cosmetic surgery.

Typically, breast implant sizes are determined by evaluating your current breast size, your body type (height, torso width), your desires and lifestyle. Generally, an increase of 180-200cc of fluid will translate to an increase in one bra cup size. We also like to look at the types of breast implants available to give us the desired results.

Immediately after breast augmentation surgery, breasts will feel very firm because of the muscle contracting over the implants. Usually within 2-3 weeks, the muscle will relax and the implants will feel softer. Breast implants generally feel natural and soft. Cohesive gel implants will feel almost identical to normal breast tissue whereas saline implants will feel slightly firmer.

Will breast implants effect breast feeding in the future?

Generally speaking, breast implants should not effect breast feeding. Incision and implant placement could make a difference in terms of breast feeding success. A breast implant placed under the muscle (submuscular) is not likely to disturb milk ducts. Implants placed on top of the muscle (subglandular) have the potential of causing problems with nursing due to the disturbance of breast tissue.

An incision made around the edge of the nipple (periareolar) may disturb the milk ducts while an incision in the crease of the breast should not. What is symmastia?

In certain areas (particularly the abdomen and thighs), we will specifically evaluate your current skin tone and advise you of possible results. Is liposuction very painful?

Yes, there is some level of soreness and/or pain associated with liposuction surgery. Patients typically tell us it feels as if they have a deep bruise or contusion in the treated areas. Generally speaking, the best placement for breast implants is beneath the pectoralis muscle or subpectoral placement. Using this placement provides more tissue coverage over the breast implants while decreasing your risk of rippling. The rate of capsular contracture is also lowered when breast implants are placed under the pectoral muscle. Only when the nipple of the breast is below the inframammary fold (breast crease) and the patient does not wish the scarring of a breast lift or mastopexy, would I consider pre-pectoral placement.

How does rippling occur in breast implants?

Breast implant rippling is a possible complication that can occur for different reasons after augmentation. Implant position (under the muscle or on top of the muscle) will play a role in the look of the breast implants after surgery. Typically breast implants placed under the muscle have a lower rate of rippling than those placed on top of the muscle.

If the patient has little muscle coverage over the ribs, the edge of the implant may appear as slight rippling to the skin. The larger the pectoral muscle, the better chance of coverage over top of the implant. The better the coverage, the less likely you are to see the edge of the breast implant.

The type of implant will also determine the chances of the implant rippling. The softer silicone gel implants typically have a decreased risk than that of saline breast implants. Also, capsular contractures can potentially cause rippling of the implants.

How often do you hear that line from a man? Well, let me be the first to utter those words to you. Truth be told, it is not your fault when it applies to the wrinkling around your mouth (perioral skin). A recent study conducted by a team of plastic surgeons in the Netherlands showed that there are gender specific differences between men and women pertaining to the nature and extent of wrinkling of the mouth area. Using the skin of cadavers and live subjects, the study found the following reasons why women's perioral wrinkles are deeper and more profound.

Women's perioral skin contains fewer sweat glands and sebaceous glands (glands that secrete an oily, waxy matter to lubricate skin and hair). This certainly may influence the natural filling of the dermis.
Women's perioral skin has fewer blood vessels therefore is less vascularized which could accelerate wrinkling.
In women, muscle fibers are more closely attached to the dermis of the mouth causing a more pronounced retraction of the skin causing....you guessed it...WRINKLES.

Filler-up

Now, that statement can refer to a multitude of things from pulling into a gas station to sitting on a bar stool with the boys from Cheers. But from a cosmetic standpoint, "filler-up" refers to a host of materials now being used to treat facial wrinkles and lines.

Injectable materials have long been a staple for facial rejuvenation dating back to bovine collagen injectables like Zyderm and Zyplast. The very first injectables often required an allergy test prior to injection and were not very long lasting.

We have come a long way. Today's fillers are longer lasting and do not need prior allergy testing. Restylane/Perlane, Juvederm, Artecol, and Sculptra (among many others) now fill the void for those people who desire a younger refreshed look without surgical intervention.

Anesthesia

Pittsburgh plastic surgery patients have many options when it comes to the aspect of anesthesia. The options can be confusing and the thought of anesthesia alone can make most cosmetic surgery patients very fearful. Well, never fear...because we are here...to help. (Yes, that was bad.) There are a variety of factors that play a role in determining anesthesia-the patient's body type, medical history, type of procedure, and extent of time in the operating room setting. Dr. James Barber will discuss your options so that you are clear on what to expect during and after surgery.

OPTIONS

Local Anesthesia
Local anesthesia is typically used for noninvasive procedures. The area surrounding the treatment site is numbed with local injections. The anesthetic can be applied as an injection in the tissue or as a "block". This approach is not recommended for larger-scale plastic surgery procedures because patients are typically fully awake and coherent. We normally use local anesthesia for procedures such as mole removals, laser skin resurfacing procedures, and facial filler treatments (upon the patient's request).

Twighlight Sedation
With this approach, there is a continuous stream of intravenous sedation which allows the patient to remain "awake" throughout surgery. The plastic surgery patient, while "awake", will not feel the procedure and will not remember the experience. We suggest twilight sedation for facial rejuvenation procedures-full facelifts, mini facelifts, eyelid lifts, and brow lifts.

General Anesthesia
General anesthesia is typically used for more invasive procedures such as nasal surgery, tummy tucks, breast augmentations/lifts, and liposuction. This approach places the patient in a sleep state without sensation for the duration of the procedure. General anesthesia carries some risks however in the hands of an experienced plastic surgeon and anesthesiologist, these risks are minimal.

THE BOTTOM LINE

Whenever you are heavily sedated or have general anesthesia, you put your life in someone's hands. Much of that responsibility falls to the person administering the medications and monitoring your vital signs. You need to know the qualifications of the person assuming that responsibility. Their training can range from specialization after earning a medical degree (anesthesiologist) to specialization after nursing training (nurse anesthetists) to nursing training only.

After years of performing cosmetic surgery procedures like breast enlargements, Dr. Barber is well aware of which approaches do--and do not--tend to work. Help with breast implant sizing

We hear it all the time-"I don't know what size breast implant to use" or "I don't want to be too big".

Implant size is a very personal decision every breast augmentation patient will have to make. Actual bra cup size is an inexact estimate of final breast size due to differences in the cup sizes of bra manufacturers. So it's not always as easy as saying "I am an "A" cup and I want to be a "C". You may be an "A" cup with a certain style/make of bra and a "B" cup in a different style/make of bra. It also depends on how you like to wear your bra (tight or loose) and the amount of breast tissue already present.

Describing your ideal cup size will aid Dr. Barber, but it also helps to look at photos and "try on" different implant sizers. These sizers are made available to you during your consultation in our Sewickley, South Side (Pittsburgh), and New Castle locations.

Most patients decide the best way to determine size is in the privacy of their own home. The most accurate method of estimating the implant size (in ounces) is to fill a plastic bag with measured amounts of water to place in your desired sized bra. You can then study the look of your new shape-in your own home, for as long as it takes, with different styles of clothing. This is a fabulous fun way to get a better idea of what your new body will look like.
